To apply for RCE Foundation’s Family Grant:

1. Complete Contact Interest / Eligibility Requirements Form Online
2. If preliminary form is approved, complete Family Assistance Program Application.
3. Application will be reviewed by the Scholarship Committee up to 4 times per year to award scholarships for families to cover services or therapeutic materials.
4. Once awarded, if therapy services are approved, the professional will submit an invoice to the Foundation for services rendered.
   1. Invoice requirements: Dates of service, brief description with ICD 10 codes and CPT codes, if applicable, and professional credentials for providing the services.
   2. It is to be noted that documentation will need to be submitted upon request of the foundation for auditing and verification purposes
   3. If materials are needed, families will submit invoices to the Foundation for reimbursement of the product(s). Family can be awarded funding to support purchase initially. Receipts and documentation will be required post purchase of materials.
5. Follow up from family. It is expected that a follow up report will be submitted to RCE Foundation on how the materials or services have or continue to benefit the client within 3 months of approval of award. For ongoing services, progress reports will be due every 3 months to ensure continued relevance and services are warranted.
